Independent laboratory coding services

An independent laboratory performs diagnostic tests and is independent of both the attending or consulting physician’s office and/or the hospital where services are rendered.

Hierarchical condition category (HCC) coding

HCC is a risk adjustment model which is used to calculate risk scores (HCC scores), which is required to adjust capitated payments made for aged and disabled beneficiaries who are enrolled in Medicare Advantage (MA) and other plans. Some providers have the resources and are good enough in risk adjustment modeling to justify all of the compliance risks that they face. This creates a problem for providers as significant dollars are at risk for their organization. HCC audits are done In order to reduce these risks and perform risk adjustment for providers.

Ambulance coding

Ambulance coding is done for patients who utilize ambulance services where the use of other methods of transportation is contraindicated by the individual’s condition, but only to the extent provided in regulations. The nature of an ambulance’s response (whether emergency or not) does not independently establish or support medical necessity for an ambulance transport. Rather, Medicare coverage always depends on, among other things, whether the service(s) furnished is actually medically reasonable and necessary based on the patient’s condition at the time of transport
